Worker Compensation
For patients whose pain or injury is work related, please provide the office with an authorization letter from your employer or worker's compensation carrier including claim number or case number, contact person and their telephone number and appropriate billing address.
Auto Claims
For patients whose injury is auto accident related please bring your insurance policy information.

There will be a $10.00 charge for completing disability forms. Please allow 10 business days for completion of the form.
